Prime Care Family Health Team has launched a regional Gender Affirming Care Clinic, and we couldn’t be more excited! This new initiative is led by a multidisciplinary team including a Nurse Practitioner, RN, RPN, Social Worker, System Navigator, and Pharmacist. This clinic is dedicated to providing gender-affirming care and creating a safe, inclusive space for everyone. We have already had many community members attend the clinic and they’ve had such positive and affirming experiences.     Halton Region is located within the traditional territory of the Anishnaabe, the Haudenosaunee, and the Huron-Wendat and the treaty lands of the Mississaugas of the Credit First Nation.

    These lands are included in the Brant Tract Treaty (No.8, 1797) and the Ajetance Treaty (No.19, 1818).
